Vrste levkemije obi\u010dajno delimo na akutne in kroni\u010dne, odvisno od hitrosti napredovanja bolezni.<\/p>\n<p>&nbsp;<\/p>\n<p>Na splo\u0161no govorimo o levkemiji ob prisotnosti biolo\u0161kih sprememb v krvnih celicah (rde\u010dih celic, belih celic in trombocitov), \u200b\u200bki povzro\u010dajo rast in nenadzorovano razmno\u017eevanje samih celic. Ime izhaja iz gr\u0161ke besede leukemia leucos = bel, ker se bolezen za\u010dne v belih krvnih celicah, ki so odgovorne za boj proti oku\u017ebam, ki se normalno razmno\u017eujejo glede na potrebe telesa.<\/p>\n<p>&nbsp;<\/p>\n<p>Pri bolnikih z levkemijo v kostnem mozgu nastane veliko \u0161tevilo nenormalnih belih krvnih celic, ki imajo genetske mutacije v DNK in zato ne delujejo pravilno. Vzroki za to bolezen \u0161e niso popolnoma razumljeni, vendar se zdi, da so vpleteni tako genetski kot okoljski dejavniki.<\/p>\n<p>&nbsp;<\/p>\n<p>Kroni\u010dne vrste levkemije so pogostej\u0161e pri odraslih. Najpogostej\u0161i obliki sta kroni\u010dna limfocitna levkemija (CLL) in kroni\u010dna mieloidna levkemija (CML): morda bi bilo primerneje uporabiti besedo &#8220;enzootska&#8221; namesto &#8220;levkemije&#8221;, glede na jasne razlike v vedenju, odzivnosti na zdravljenje in napoved iz akutne oblike levkemije.<\/p>\n<p>&nbsp;<\/p>\n<p>KML je zelo pogost pri odraslih in zelo redek pri dojen\u010dkih, s srednjo starostjo za\u010detka okrog 60 let in incidenco v populaciji pribli\u017eno 2,3 nova primera na leto na 100 000 prebivalcev, v o\u010ditni rasti glede na povpre\u010dno pove\u010danje \u017eivljenjske dobe.<\/p>\n<p>&nbsp;<\/p>\n<p>Glavna zna\u010dilnost te bolezni je nenormalno razmno\u017eevanje zrelih belih krvnih celic in redkeje trombocitov, ki jih najpogosteje najdemo naklju\u010dno ob rednih kontrolnih pregledih. V preteklosti, ko je bila redkej\u0161a preiskava, je diagnoza CML izhajala iz povratnih informacij o ob\u010dutku te\u017ee na levi strani zaradi rasti velikosti vranice (splenomegalija), ki je \u0161e vedno povezana z naprednej\u0161imi fazami od CML.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h2 id=\"kaksni-so-izvori-te-bolezni\">Kak\u0161ni so izvori te bolezni?<\/h2>\n<p>&nbsp;<\/p>\n<p>Leta 1960 je bila izvedena \u0161tudija kromosomov (kariotipizacija), ki je pripeljala dva zdravnika iz Filadelfije in ugotovila, da imajo bolniki s CML kromosom 22 kraj\u0161i od obi\u010dajnega. Ta oblika &#8220;kratkega&#8221; kromosoma 22 se je imenovala Philadelphia kromosom (Ph kromosom). Kasnej\u0161e \u0161tudije so pokazale, da se je pri bolnikih s CML kromosom 9 izkazal tudi druga\u010de. Ugotovljeno je bilo, da je med kromosomom 9 in kromosomom 22 vzajemna izmenjava kromosomskega materiala: in sicer, da je del kromosoma 9 povezan s kromosomom 22 in obratno (translokacija). Rezultati so bili, da so bile vse levkemi\u010dne celice pozitivne na translokacijo 9 in 22, ostale celice kosti in tkiva pa tiste z obi\u010dajnim kariotipom.<\/p>\n<p>&nbsp;<\/p>\n<p>V 80-ih letih je bilo znano, da je premestitev 9; 22 je vklju\u010devalo predvsem dva gena &#8211; gen BCR (kromosom 22) in gen ABL (kromosom 9) &#8211; ki je povzro\u010dil tako imenovani fuzijski gen BCR-ABL. Slednje povzro\u010da nenormalno beljakovino (ki je ni prisotna pri zdravih ljudeh), ki pomembno vpliva na celi\u010dni cikel, ki levkemi\u010dne celice privede do slabo nadzorovane proliferacije.
