Il s&#8217;agit d&#8217;un r\u00e9seau lin\u00e9aire de trois nodules maintenus ensemble par un fil tr\u00e8s mince dont le diam\u00e8tre est estim\u00e9 entre 8 et 15 Angstrom. C&#8217;est une glycoprot\u00e9ine synth\u00e9tis\u00e9e par le foie qui joue un r\u00f4le important dans le processus de coagulation sanguine. Pendant la coagulation normale du sang dans le corps humain, une cascade de coagulation active la prothrombine zymog\u00e8ne en la transformant en thrombine s\u00e9rine prot\u00e9ase. La thrombine transforme ensuite le fibrinog\u00e8ne soluble en un brin de fibrine insoluble. Ces brins sont ensuite r\u00e9ticul\u00e9s par le facteur XIII pour former un caillot sanguin si n\u00e9cessaire dans l&#8217;organisme.<\/p>\n<p>Une carence cong\u00e9nitale en fibrinog\u00e8ne ou un dysfonctionnement du fibrinog\u00e8ne peut entra\u00eener des saignements ou des complications thromboemboliques, ou se r\u00e9v\u00e9ler non pathologique sur le plan clinique. La carence acquise est constat\u00e9e apr\u00e8s h\u00e9modilution, pertes sanguines et\/ou consommation, comme chez les patients traumatis\u00e9s, pendant certaines phases de la coagulation intravasculaire diss\u00e9min\u00e9e (CIVD), ainsi que dans les septic\u00e9mies. Chez les patients atteints d&#8217;une carence en fibrinog\u00e8ne, la correction des saignements est possible par perfusion de plasma frais congel\u00e9 (PFC), de cryopr\u00e9cipit\u00e9 (une fraction plasmatique riche en fibrinog\u00e8ne) ou par des concentr\u00e9s de fibrinog\u00e8ne.<\/p>\n<p>&nbsp;<\/p>\n<h2 id=\"pourquoi-mesurer-le-taux-de-fibrinogene\">Pourquoi mesurer le taux de fibrinog\u00e8ne ?<\/h2>\n<p>Le fibrinog\u00e8ne peut \u00eatre utile pour r\u00e9v\u00e9ler des d\u00e9fauts de coagulation, il est souvent utilis\u00e9 lorsque d&#8217;autres tests de coagulation (p. ex. temps de prothrombine, temps de thromboplastine, temps de thromboplastine partielle activ\u00e9) peuvent donner des r\u00e9sultats anormaux. Les taux de fibrinog\u00e8ne peuvent \u00eatre mesur\u00e9s dans le sang veineux. Dans des circonstances typiques, le fibrinog\u00e8ne est mesur\u00e9 en laboratoire dans des \u00e9chantillons de plasma citrat\u00e9, mais l&#8217;analyse d&#8217;\u00e9chantillons de sang total par thrombo\u00e9lastom\u00e9trie (la fonction plaquettaire est inhib\u00e9e par la cytochalasine D) est \u00e9galement possible. Des taux plus \u00e9lev\u00e9s sont, entre autres, associ\u00e9s aux maladies cardiovasculaires et peuvent \u00eatre \u00e9lev\u00e9s dans toute forme d&#8217;inflammation, car il s&#8217;agit d&#8217;une prot\u00e9ine en phase aigu\u00eb ; par exemple, elle est particuli\u00e8rement apparente dans le tissu gingival humain pendant la phase initiale de la maladie p\u00e9riodontale. Les taux de fibrinog\u00e8ne augmentent pendant la grossesse pour atteindre une moyenne de 4,5 g\/l, comparativement \u00e0 une moyenne de 3 g\/l chez les femmes non enceintes.<\/p>\n<p>Les taux normaux de fibrinog\u00e8ne se situent entre 200 et 400 mg\/dL. Cependant, ces valeurs d\u00e9pendent du laboratoire et des m\u00e9thodes utilis\u00e9es. Les r\u00e9sultats anormaux peuvent \u00eatre li\u00e9s \u00e0 une consommation excessive de fibrinog\u00e8ne par l&#8217;organisme, comme dans le cas de la coagulation intravasculaire diss\u00e9min\u00e9e (CIVD), ou \u00e0 une carence en fibrinog\u00e8ne qui peut \u00eatre acquise d\u00e8s la naissance ou apr\u00e8s. D&#8217;autres causes peuvent \u00eatre dues \u00e0 la d\u00e9gradation de la fibrine (fibrinolyse) ou \u00e0 un saignement excessif (h\u00e9morragie).<\/p>\n<p>&nbsp;<\/p>\n<h2 id=\"norme-de-preparation\">Norme de pr\u00e9paration<\/h2>\n<p>L&#8217;\u00e9chantillonnage se fait habituellement le matin \u00e0 l&#8217;h\u00f4pital. Aucune pr\u00e9paration particuli\u00e8re n&#8217;est n\u00e9cessaire pour ce test. Le m\u00e9decin vous conseillera et vous recommandera si vous devez \u00eatre \u00e0 jeun avant l&#8217;examen sanguin. Vous devriez informer votre m\u00e9decin de tout m\u00e9dicament que vous prenez avant l&#8217;examen, car certains traitements m\u00e9dicaux peuvent interf\u00e9rer avec les r\u00e9sultats sanguins.<\/p>\n<p>&nbsp;<\/p>\n<h2 id=\"lexamen-est-il-douloureux-ou-dangereux\">L&#8217;examen est-il douloureux ou dangereux ?<\/h2>\n<p>L&#8217;examen n&#8217;est ni douloureux ni dangereux.
